This role involves providing high-quality, person-centred care to individuals living with disabilities and supporting them to achieve independence, dignity, and improved quality of life.

Task & responsibilities

• Providing personal care including bathing, dressing, grooming, and toileting 
• Assisting clients with mobility, transfers, and use of mobility aids 
• Supporting daily living activities such as meal preparation and feeding 
• Assisting with medication administration in accordance with care plans 
• Monitoring health conditions and reporting changes to supervisors 
• Providing emotional support, companionship, and social engagement 
• Assisting clients with community access, appointments, and activities 
• Maintaining accurate records and progress notes 
• Ensuring a safe, clean, and supportive environment 

Qualification & experience

• Certificate III or IV in Individual Support, Disability, or Aged Care (preferred) 
• Experience of Minimum 1-3 years in aged care, disability support, or community services 
• Understanding of person-centred care and NDIS practices 
• Strong communication and interpersonal skills 
• Ability to work independently and within a team 
• Current First Aid and CPR certification (desirable) 
• National Police Check and relevant clearances
