An exciting opportunity exists for an experienced Nurse or Allied Health Professional to join Western Health's Community-Based Transition Care Program in a Case Management role.

We are seeking a dedicated and skilled Case Manager to join our Community-Based Transition Care Program (TCP), a critical role in facilitating the smooth transition of clients from hospital care to sustainable, long-term living arrangements.
In this position, you will collaborate closely with clients, families, and multidisciplinary teams to assess individual needs and coordinate the delivery of appropriate community services. Your expertise will ensure that clients receive tailored support, advocate for necessary services to enable safe and efficient discharges, and provide ongoing support and service delivery education and connections across our hospital network.
As a Case Manager, you will hold both a clinical caseload and non-clinical responsibilities, playing an essential role in improving hospital flow, providing quality care to complex clients, support the exisiting case managment and wider TCP team, and contributing to the development of service strategies. Your work will directly support the enhancement of healthcare delivery through effective communication, pathway connections, comprehensive case management, and a commitment to driving quality.
Skills & Experience
Qualification in Nursing or Allied Health and registration with the relevant professional body (as required) Demonstrated experience in Case Management Demonstrated experience working with a multidisciplinary team Experience in complex discharge planning from a hospital setting Experience working with non-acute/community service providers and residential aged care facilities Current understanding of the Transition Care Program and experience with My Aged Care (MAC) to facilitate access to ongoing services, assessments, and accommodation pathways Excellent written and verbal communication skills Excellent organisational and time management skills Experience in student supervision Knowledge of and participation in continuous quality improvement processes Commitment to continuing professional development A current Victorian Driver's Licence Computer literacy
