Job Role
The Support Coordinator  is responsible for assisting NDIS participants in implementing all aspects of their NDIS plans, including linkages to services, case coordination, monitoring, implementing and reporting on all aspects of their welfare, working with informal, formal and mainstream community services. As Support Coordinator, you will work with varying age groups including children youth adult, family and multidisciplinary team. 

The role involves liaising with community mainstream services, housing, welfare, health and others identified in line with client’s care needs. You will work with your clients and their expanded network to ensure your participants' care needs do not deteriorate and they are managed in the community for consistency of services. working with other Allied Health Professionals, you will work independently to coordinate their supports, seek supervision and achieve greater autonomy. The role also include reports writing and preparing strong evidence based submission to support ongoing funded and care supports for your clients. It also includes creating comprehensive reports on outcomes and success indicators in accordance with NDIS reporting requirements.


Key responsibilities include sourcing resources, connections to NDIS services, and regular home visits. This role is focused on achieving clear participant outcomes by establishing quality frameworks and services for participants with complex needs.

Your strong stakeholder management skills and ability to help navigate through a process will be critical to your success in this role.


Key deliverables for this role are to provide resources and connections with NDIS services and NDIS information for their allocated NDIS participants as a Support Coordinator.

Job Responsibilities
• Conduct comprehensive client assessments to identify their needs, barriers, and goals.
• Facilitate community development initiatives to promote engagement and collaboration.
• Develop and implement collective solutions to address community issues, including recreational activities, health services, housing, employment opportunities, and other welfare-related matters.
• Identify and assess community needs to create targeted programs and initiatives.
• Collaborate with stakeholders to enhance community well-being and address specific challenges effectively.
• Develop and implement service plans tailored to individual circumstances.
• Support participants to navigate and maximize their NDIS plans by coordinating appropriate support services.
• Build relationships with participants, families, and caregivers to ensure effective collaboration.
• Facilitate access to community resources and services that enhance client well-being and social inclusion.
• Advocate for participants to ensure they receive fair and equitable services.
• Monitor and review client progress, adjusting plans as necessary to meet evolving needs.
• Maintain accurate, up-to-date case notes, records, and mandatory reporting requirements in line with NDIS Practice Standards.
• Liaise with multiple stakeholders, including NDIA, NDIS Quality and Safeguards Commission, service providers, and other community organizations.
• Provide crisis intervention and conflict resolution as required.
• Ensure compliance with all legal, ethical, and professional standards.

Qualification & Experience

Desired Skills 
• A thorough understanding of the NDIS, including relevant policies and Practice Standards.
• Demonstrated experience in case management, assessment, referrals, and planning.
• Strong ability to work with individuals experiencing complex needs, including mental health challenges and psychosocial disabilities.
• Excellent interpersonal and communication skills for interacting with diverse stakeholders.
• Proven ability to work autonomously, manage competing priorities, and solve problems effectively.
• Strong organizational and time management skills to meet deadlines and performance targets.
• Knowledge of local service providers and community resources.

Experience
• A minimum of 1-2 years of experience in a similar role within the disability or community services sector.
• Demonstrated experience working with individuals with disabilities and complex needs.

Qualifications
• Bachelor or preferably, Master’s degree in Social Work, Community Services, Psychology, or a related field (Essential).
• Accreditation as a Social Worker (Highly Desirable).
• Current Driver’s License and reliable access to a vehicle (Essential).
• Valid NDIS Worker Check, Working with Children Check, and National Police Clearance (Essential).
• Full Comprehensive Car Insurance (Essential).

Other Requirements
• Valid Disability and Child-Related DCSI Clearance (Essential).
• Valid Working with Children Check and National Police Check (Essential).
• Flexibility to travel between various locations as required.
