Hospital in the Home is an acute care service across the Northern Sydney Local Health District (NSLHD) caring for stable patients in their home. We are looking for senior registered nurses with strong acute care clinical skills and strong communication skills to join our multidisciplinary team. Where you'll be working Mona Vale Hospital - Hospital in the Home The role is primarily based at Mona Vale Hospital however the incumbent will be expected on occasion to start at one of our other sites – Royal North Shore Hospital or Hornsby Hospital or Northern Beaches Hospital. What You'll Be Doing As an independent practitioner, the Registered Nurse (RN) will be caring for acute patients in their home, escalating any concerns or deterioration, communicating to the multidisciplinary team, and providing holistic care. The RN must have excellent assessment skills and confidence to problem solve in unfamiliar environments. As a flexible RN. you will at times be asked to be team leader, hospital liaison, or work in our clinics across the service. Demonstrates these behaviours with all stakeholders; colleagues, direct reports, as well as our patients and consumers, and those that care for them.Registered Nurse –Division 1 with Australian Health Practitioner Regulation Agency (AHPRA).Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.Sound experience using electronic medical record systems and office applications, and evidence of personal time management capabilities and effective negotiation skills.Current NSW Drivers Licence.
