Nurse Practitioner – Bupa Aged Care (Work from home)

Location: Work‑from‑home with possible travel to aged‑care homes in your hub
Reports to: Medical Services Manager
Employment: Full‑time or Part-Time, Permanent

What You’ll Do

• Wellness hubs: Lead a multi-disciplinary team in collaborative discussions about residents that have been referred to the hub.
• Telehealth: Review residents referred to NP. This will include ALL transitions of care: admissions and hospital discharges, advance care planning and progress reviews.
• Collaborative Care: Coordinate with multi-disciplinary team members such as GPs, allied‑health and care‑home staff to deliver person‑centred plans.
• Education & Mentoring: Coach nursing staff including NP candidates; contribute to policy and quality‑improvement projects.
• Governance & Research: Participate in clinical governance and research initiatives.

What We’re Looking For

• Mandatory:
  • Bachelor of Nursing + Master of Nursing (NP) or equivalent
  • Current NMBA registration & NP endorsement
• Desirable:
  • Aged‑care or primary‑care NP experience
  • Dementia, palliative care & advance‑care planning expertise
  • Medicare billing and electronic‑records proficiency
