We’re looking for a dedicated Outreach Officer to help us expand our reach, foster meaningful partnerships, and promote our mission to new audiences. This is a unique opportunity to play a key role in growing our presence and making a real impact through outreach and engagement initiatives. This is a Full-Time permanent role working Monday to Friday with up to 6 weeks leave per year.

Your Contribution

The Outreach Officer is responsible for leading outreach initiatives aimed at strengthening relationships between Indigenous communities and service providers. This role involves coordinating programs and activities that promote awareness, understanding, and access to resources for Indigenous peoples. The Outreach Officer will work closely with Health Workers, organisations, and stakeholders to ensure that outreach efforts are culturally appropriate and responsive to the unique needs of Indigenous communities.

Duties and Responsibilities

  • Provide health education to the Aboriginal and Torres Strait Islander Community
  • Work in Collaboration with TAIHS Primary Health services for outreach programs that connect Indigenous individuals and families with essential services such as healthcare, housing, education, and legal assistance
  • Coordination of Rheumatoid Heart Disease program recalls and provide health education to community to provide advice around the prevention and management of symptoms relating to RHD
  • Working alongside Health Professionals to provide home visits and increase the uptake of compliance with Bicillin Therapy, Echo monitoring and Cardiology review to improve access to primordial prevention
  • Collaborate with internal teams, government departments, and external partners to ensure effective coordination and delivery of services, with a focus on addressing gaps in services or access
  • Coordinate all Qld Health Services attending TAIHS including Cardiologists, Endocrinology and Chronic Disease specialists
  • Oversee and assist patient documentation, diagnostic services, the planning and management of patient care systems and processes, patient advocacy

Required Qualifications and Experience

  • Certificate IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent
  • Current First Aid and CPR Certification
  • Proven ability to work as part of a multidisciplinary team and to work independently as required
  • Knowledge of the issues affecting the health and well-being of Aboriginal and/or Torres Strait Islander people living in the Townsville region
  • Knowledge of MS Office Suite, Best Practice, with the ability to use other software systems relevant to the Primary Health Service environment
  • Comprehensive written and verbal communication skills
  • Awareness of culturally safe practices with an awareness of cultural protocols
