About The Role
An opportunity currently exists for a Grade 2 Registered Nurse to join the Transition Care Team on a full time or part time, permanent basis.
As a Grade 2 Registered Nurse you will be responsible for the delivery of high-quality patient care within Ellerslie Unit’s supportive team environment as well as ensuring a safe environment for both patients and staff.
Duties include:
Practices in accordance with the professional standards, codes and behaviours that are regulated through the Nursing and Midwifery Board of Australia (NMBA)Works collaboratively within scope of practice to ensure the efficient, effective and responsive operation of the unitSupports and encourages patients and their families understanding of care delivery and care planning through effective and appropriate communication Ensures clinical interventions are evidence-based and consistent with policy, standard and clinical practice guidelines prescribed by SVHMAccountable for timely communication with the nursing team, multidisciplinary team and reporting outcomes of nursing interventions to the Nurse Unit Manager or delegateTreats all patients, visitors and staff in a courteous and non-discriminatory manner and maintains confidentiality in regards to patients and all staffTakes responsibility for managing own learning needs and participates in relevant professional development programs including contribution to ongoing education initiatives within the work unitComplies with mandatory and other identified training requirements as specified by the NMBA & SVHMComplies with SVHM policies and reports all actual or near miss events Participates in the supervision, support and mentorship of students and nursing staff as directed by the Nurse Unit Manager or delegateProvides supervision and appropriate clinical delegation to the Enrolled Nurse, Health Assistant and pre-registration learnersPromotes a culture of continuous quality service improvement within the unit and participates in activities designed to continuously improve standards of service and practice Participates in any portfolio responsibility as delegated by the NUMSupports the NUM or delegate in the identification of initiatives to improve sustainability in TCP and contributes to a positive and supportive environment.
Classifications for the RN position will range from YP3 - YP11 ($65,276 - $87,282 per annum, pro rata for part time).

Current Nursing Registration with AHPRACompleted Graduate Nurse Program or equivalentCurrent experience in Subacute or Aged Care nursing is desirableDemonstrated competent clinical nursing skillsDemonstrated ability to facilitate team outcomes in multi-disciplinary environmentExceptional interpersonal skills with patients, visitors and staff at all levelsCurrent national criminal history check, or willing to obtain

About The Department
The SVHM Bed Based Transition Care Program (BB TCP) provides person centred care, case management and restorative services for elderly clients, who are cared for by a skilled and compassionate multidisciplinary team. The BB TCP is located at Ellerslie Unit, St George’s Health Service in Kew.
