This is an exciting leadership opportunity for a highly motivated, experienced Nursing or Allied Health professional to join the Western Health at Home team as a Service Manager for the Aged Care Assessment Service.
The Aged Care Assessment Service provides comprehensive assessment, information and advice to frail older people and their carers and facilitates access to available care services appropriate to their needs.
Aboriginal people and/or Torres Strait Islanders are eligible for an ACAS assessment from 50 years of age.
