Service Manager - Aged Care Assessment Service

Full Time, Permanent (1.0 FTE)

This is an exciting leadership opportunity for a highly motivated, experienced Nursing or Allied Health professional to join the Western Health at Home team as a Service Manager for the Aged Care Assessment Service.

The Aged Care Assessment Service provides comprehensive assessment, information and advice to frail older people and their carers and facilitates access to available care services appropriate to their needs.

Aboriginal people and/or Torres Strait Islanders are eligible for an ACAS assessment from 50 years of age.

Key Focus:

• Independent assessment of physical, medical, psychological, environmental and social needs
• Linking people with community services
• Providing support and access to people requiring transition care programs or residential aged care

The Aged Care Assessment Service is looking for candidates that:

• Are experienced leaders
• Possess strong communication and collaborative teamwork skills
• Adaptable and demonstrate highly developed problem solving and lateral thinking skills
• Will act as a role model and mentor for the team

To be Successful in this Role you must:

• Have appropriately recognised tertiary qualification
• Demonstrate highly developed leadership skills and experience leading teams
• Demonstrate highly developed communication and interpersonal skills
• Ensure safe and high-quality patient care is delivered in line with Western Health values and Live Best Care framework
• Contribute to a culture that values ongoing learning and be part of a positive workplace
• Have a valid Victorian Drivers Licence
