The primary focus of this role is to ensure participants are fully satisfied with their service, facilitate seamless delivery of complex programs with strong support worker engagement, and help participants stay on track to achieve the goals outlined in their NDIS or Care Plans.

About the role:
Providing the coordination of supports to complex programs in allocated portfolio; supporting the individual’s ability to connect with informal, mainstream, and funded supports, and to increase capacity to maintain support relationships, and resolve service delivery issues.
Collaborate with participants to develop, implement, and monitor individualized support plans.
Support NDIS participants and families in accessing and coordinating services to achieve their goals.
Coordinate and connect participants with appropriate service providers to address their needs.
Advocate for participants to ensure they receive the necessary support and resources.
Monitor the progress of participants' goals and adjust support plans as needed.
Build and maintain relationships with internal and external stakeholders, including service providers and allied health professionals.
Complete all necessary administration, ensuring full compliance with NDIS requirements.
Maintain accurate records and report on participants' outcomes.
About You:
2+ years of experience in a similar role, supporting participants with mental health, psychosocial, or disability support needs.
Relevant tertiary qualifications in disability or a related discipline.
You have a strong understanding and knowledge of the disability support sector, including relevant legislation, funding models, and person-centered support practices.
Your excellent communication and interpersonal skills allow you to build trust and rapport with participants, stakeholders, and colleagues.
You are empathetic, committed to making a positive impact in people’s lives, and can think creatively to solve problems and meet participants' needs.
You possess strong organizational and time management skills, with ability to thrive in a fast-paced customer service environment including the ability to manage multiple and competing priorities.
You are skilled in engaging potential clients, maintaining stakeholder relationships, and working autonomously and as part of a team, taking responsibility for assigned tasks and actively contributing to team meetings.
Have a valid Driver’s licence and reliable vehicle.
Prepared to undergo Safety Screening requirements including Police Check, NDIS Worker Screening Check, and provide a current Working with Children Check.
Current First Aid and CPR.
