Ahanacare is looking for an experienced and exceptional Welfare Support Coordinator on a FULL-TIME basis to join our team at Morley, Western Australia.

As a Welfare Support Cordinator, you liaise with all support / care team members and provide individuals coordinated care and improve quality of live.  You will also work closely with external stakeholders such as welfare and community service agencies, private entities and government bodies to develop client care plan and manage quality activities.

Task & Responsibilities

  • Report to the Service Manager or Director.
  • Liaise with all members of the support/care team and provide coordinated care.
  • Carry out comprehensive and accurate client assessments.
  • Contribute to the development, implementation, monitoring and review of care plans to meet client’s care needs and to promote quality of life.
  • Promote a consultative and collaborative approach to client care planning and quality-managed activities with external stakeholders such as welfare and community service agencies, private entities, and government bodies (including Service Australia and NDIS)
  • Monitor and report on the client’s progress.
  • Maintain appropriate records and documentation.
  • Support and assist clients in life skills development, mental well-being and reaching individual goals in the community. 
  • Refer clients to appropriate agencies for additional help if necessary.
  • Assist with the orientation of new staff and ongoing professional development of staff.
Qualification & Experience

Essential Skills and Qualification:

  • Current First Aid certification
  • Relevant advanced diploma or higher qualification in community service or community development
  • Demonstrated knowledge and skills in assessment, treatment and evaluation
  • Demonstrated ability to work effectively in a multidisciplinary team setting as well as autonomously
  • Ability to work with people from diverse backgrounds with compassion and empathy and without judgement for their lifestyle choices, personal preferences and values
  • Demonstrated experience and knowledge of NDIS and working within a legislative framework

 

Desirable:

  • Member of ACWA is desirable
  • A can-do attitude, enthusiasm and great communication skills
  • Strong, proven interpersonal skills and empathy will be found in the right candidate
  • Excellent prioritisation and organisational skills
  • Proficient in a 2nd language other than English
