Task & responsibilities
About the Role
As a Disability Services Officer, you will work closely with participants, families, carers, support workers and external service providers to coordinate and deliver quality disability support services.
Your responsibilities will include:
- Developing and reviewing person-centred support plans.
- Coordinating supports in line with participants' goals and NDIS funding.
- Monitoring service delivery to ensure quality outcomes.
- Conducting assessments and identifying participant needs.
- Liaising with families, healthcare professionals and community organisations.
- Maintaining accurate case notes and participant records.
- Supporting participants to access community, education, employment and recreational opportunities.
- Ensuring services comply with NDIS Practice Standards and organisational policies.
- Identifying risks and implementing appropriate safeguarding measures.
Qualification & experience
About You
To be successful in this role, you will have:
- Qualifications in Disability, Community Services, Social Work, Human Services or a related field (desirable).
- Experience working within disability, community services or aged care.
- Sound knowledge of the National Disability Insurance Scheme (NDIS).
- Excellent communication and interpersonal skills.
- Strong organisational and problem-solving abilities.
- A commitment to person-centred, inclusive and culturally safe practice.
- The ability to work independently and as part of a multidisciplinary team.
Essential Requirements
- Current National Police Check.
- NDIS Worker Screening Check (or willingness to obtain).
- Working With Children Check (where required).
- Current First Aid and CPR Certificate.
- Current unrestricted Australian Driver Licence.
- Full Australian working rights.
